  12.     (yes) Type in "Reference Number" area you number, up to
  13.     24 characters will appear on your bill.
  14.     (no)  You may leave the "Reference Number" space blank.

  19.     You will pay for shipping charges that will be charged to your

  22.     Charge the recipient the cost and put their Account number on
